Our easy-to-use Patient Portal allows you to update your records conveniently from anywhere, whether you’re on a computer, laptop, tablet, or smartphone.

 

It is essential that you complete your medical history and check your personal details before arriving for your upcoming appointment, including full details of any medication you are currently taking.  As we have recently changed our software, you are required to complete a new form. Going forward, your details will be saved in the Patient Portal, and you will be able to check and update your medical history form as required.

 

If you don’t have access to a smartphone or computer, don’t worry—we’ll have iPads available on site for you. Just remember to arrive about 20 minutes early for your appointment so you have enough time to comfortably fill out all the forms.
