The portal is very important, as it provides contactless interaction prior to your appointment to complete all the necessary documentation, thereby enabling us to spend more time with you in surgery. It has a secure link to your booking. When you click on the link, you will be able to update and fill in forms related to your medical history, personal details, GDPR, terms and conditions and NHS forms. You will also be able to fill in an oral health questionnaire, which will help us to tailor your dental needs to suit you. These portal forms can be filled in on a computer, laptop, tablet or smart phone.
You will also receive a text message the day before your appointment, with a new link to the patient portal. Please log into the portal again and complete our COVID-19 health screening questions.

For those who do not have access to this type of technology, we will provide iPads (which will be wiped clean after each use) on site. You will however need to arrive 20 minutes before your appointment to allow suitable time to fill in all the forms in this manner.
You will also get a QR code which you can use to check in for your appointment. You will only be able to use this check in code 30 minutes prior and up to 5 minutes after your booked appointment time.
